Curtis Crook is a physical therapist of 17 years specializing in Orthopedics and Sports Medicine. Curtis graduated from University of Maryland with his BS in Kinesiology and Western University of Health Sciences with his Masters in Physical Therapy. He did a Post
Graduate Orthopedic residency for 1 year after graduating with his Physical Therapy Degree. Early in his career he practiced in Southern California where he was a clinical director for an outpatient physical therapy practice. He was on the medical staff for Asterisk Medical providing medical coverage and care for Supercross and Motocross athletes. He was also on the medical staff for Vista Murrieta High School providing medical coverage as and care to the high school athletes. Curtis helped develop an electronic medical record (EMR) called Optimis PT which we currently use in our Top Tier Sports Medicine clinics. Curtis started with Top Tier Sports Medicine 7 years ago coming on to manage the Sports Medicine Trailer on the Ladies Professional Golf Tour. Curtis has been the Physical Therapist for the last 3 USA Solheim teams. Curtis worked the 2016 Summer Olympic Games in Rio working specifically with the male and female golfers. Following the Olympics Curtis started providing medical care for the rock band Korn, he has toured with the band for the past 3 years all over the world ensuring they are in good health, functioning physically at their best. In the past year Curtis opened Top Tier Sports Medicine’s 3rd location in Indialantic, Florida. When he is not working he enjoys spending time with his family wife Heidi and son Christian. Curtis enjoys working out at Trinity Fitness, Cycling and attending church at Coastline Church. Curtis lives in Satellite Beach, Florida and enjoys spending time at the beach with his family.

Cherri earned a B.S. in Physical Therapy from The Ohio State University. She has been practicing physical therapy for over 33 years in the Melbourne area. She earned certification in Neurological Developmental Training for adult
hemiplegia and has extensive experience working with neck and back disorders. She specializes in the treatment of Bells Palsy, Vertigo/Dizziness, and Balance Disorders in the elderly.

Paige Hubert received her Bachelors of Science in Psychology with a minor in Physical Science from Troy University in 2015. She received her Doctorate of Physical Therapy from Texas Woman’s University in 2018. Paige moved from Dallas, Texas in 2018. She enjoys treating pelvic floor dysfunction, pediatrics, orthopedics, and chronic pain patients. She wants to ensure a fun, effective, and safe environment to ensure highest patient satisfaction and healing.
Pamela Gates

Pamela Gates received her Bachelors of Health Science degree in 2007 and her Doctorate of Physical Therapy in 2010, both from The University of Florida. While in college, she was active in the marching band, concert band, and played ultimate frisbee. Pamela has been working in outpatient orthopedics in the Melbourne area since 2010. She has received ongoing training for TMJ disorders and vestibular disorders. She is a Graston Technique Specialist since 2015 and a Certified Kinesio Taping
Practitioner since 2012. On her free time, she enjoys college football, gardening, kayaking and spending time with her husband and son.
